This module is only to used to request Marine Corps pay records.  In order to process any request for pay records, a Record Services Request form must be completed and attached to the askDFAS ticket.  The completed form must be signed and a scanned copy or photo of a valid not expired government issued ID card must be included for identity verification. Identify your duty status component(s) during the period for which pay records are being requested.
Pay Entry Base Date in MM-DD-YYYY format. For example 12/31/2000
End of Service identifies the date for which the member will be, or was, discharged from active duty or reserve military service. Enter the date you were or will be discharged from your most recent military service status; Active Duty or Reserve.
Enter Rank. For example SGT, Major, etc.
 
In order to complete this form, you MUST attach a photo copy of your ID
for verification in the below attachment section.
