In order to process any request for pay records, a Record Services Request form must be completed and attached to the askDFAS ticket.  The completed form must be signed and a scanned copy or photo of a government issued ID card must be included for identity verification. Identify your duty status component(s) during the period for which pay records are being requested.
Pay Entry Base Date in MM-DD-YYYY format.  For example 01-31-2018
End of Active Service identifies the date for which the member will be, or was, discharged from military service.
Enter Rank. For example SGT, Major, etc.
