Create a passcode for this ticket to view information and responses. Passcodes must be a minimum of 15 characters and contain at least one lowercase letter, one uppercase letter, one number, and one special character. This online upload tool is for submitting SF 1174 forms and documentation to claim Arrears of Pay for deceased military retirees. Do not submit documentation without an SF 1174 form using this tool.

BEFORE YOU UPLOAD, MAKE SURE YOUR FORM IS SIGNED AND DATED. Forms without signatures or dates will cause delays in processing.

ONLY the SF 1174 and accompanying documents can be submitted using this tool. DO NOT attempt to use this tool to submit other requests and documents, this will cause a delay in processing.

Submits your ticket and file(s) to DFAS Retired & Annuitant Pay. Please allow a minimum of three business days for your file(s) to be indexed in our system. Also note: your Ticket number CANNOT be used as a reference number for status requests if you call our Customer Care Center.
Cancels your inquiry and returns you back to the FAQ Page.
