Payment Not Received

Question

A payment was issued to me more than 30 days ago but I have not yet received it, what should I do?

Answer

If you are still in service, please contact your unit/command.  If you are separated and a payment was issued and you did not receive it, complete DD Form 2660 Claimant Requesting Replacement Check and send to:  

Disbursing Operations, Indy Check Team, 8899 E. 56th St, Indpls, IN  46249 or fax to 317-212-2994. 

The form can be found at the link below.  Once you receive a response from the US Treasury, please create a new ticket and forward the documents.
