What information/documentation is required when submitting a base level debt?

Question

I have a base level debt to submit to your office and would like to know what is required to establish the debt?

Answer

Eligibility -
     * Debtor has an individual debt
     * Debtor is currently listed as separated on all DoD pay systems
     * Debt is delinquent
     * Total debt is more than $225. or multiple debts add up to more than $225.

Transmittal Letter (TL)
     * Guidance provided in the DoD FMR Volume 16, Chapter 3, Section 030504
     * Debtor's full name, COMPLETE SSN, and debt amount(s)
     * Debt Originator's POC information: Name, Email Address, and Phone Number

Certification Statement -
     * Guidance provided in the DoD FMR Volume 16, Chapter 3, Section 030504
     * Copy of one found on page 3-40 of the DoD FMR Volume 16 Chapter 3

Due Process Demand Letter -
     * Guidance provided in the DoD FMR Volume 16, Chapter 3, Section 030504
     * Provide a minimum of one due process demand payment letter

Line of Accounting (Accounting Classification) -
     * Provide the complete Line of Accounting(s) and year(s) to be credited with any collections

Substantiating Documents/Information -
     * Guidance provided in the DoD FMR Volume 16, Chapter 3, Section 030504
     * The original debt principal amount
     * The fill name and complete SSN
     *Examples of documents supporting debt validity and providing detailed information : 
          - Copy of disbursement voucher when the debt is the result of a previous payment
          - Travel Voucher including full name, complete SSN, and debt amount
          - DD Form 139
          - DD Form 2481
          - GPL/FLIPL: DD Form 200 for debts due to Financial Liability for Government 
             Property Lost, Damaged, Destroyed, or Stolen
          - Tuition Assistance: List of class description, institution, dates, debt reason
          - Government Quarters:  Address of quarters, dates, description of damage or rent due
          - Military Treatment Facility: List of all invoices including fill name, complete SSN, and debt amount
