What if I don't know my CAC or PIV PIN or my CAC or PIV is expired or revoked?

Answer

DoD CAC users, please contact your RAPIDS Station Point of Contact, your Personnel Center or the DMDC Help Desk at (800) 538-9552 to have your records corrected.  You can still access myPay using your Login ID and Password until the records are corrected.   To locate the nearest ID card office you can use the ID Card Office Online (https://idco.dmdc.osd.mil/idco/). 

HHS PIV users:
 	General questions about PIV Smart Cards and HSPD-12: Contact HSPD-12@HHS.gov.
 	AMS Login: Contact the ONE DHHS Help Desk at 1-888-ONE-DHHS (1-888-663-3447). Or email AMSHelp@hhs.gov
 	PIV Smart Card: Contact your local security office for issuance or troubleshooting.
 	Smart Card Reader: Contact your local IT Service Desk or go to the "Need Help?" link on the AMS login page to access IT Service Desk contact information.

VA PIV users:
 	Please contact the Enterprise Service Desk at 1-855-673-HELP (1-855-673-4357) if you are having trouble using your PIV or USA SMART card.
 	You may also visit your local badge office.

DOE PIV users:
 	The HSPD-12 Credential contains electronic data on a computer chip that is protected by Personal Identification Number (PIN) that you create when you get your HSPD-12 Credential  The PIN is a "6" to "8" digit number that is only known to you.
 	If your HSPD-12 Credential has been locked or you do not know what your PIN is, you must visit an HSPD-12 Activation Station and reset the PIN to something you will remember. DOE has Activation Stations throughout the complex at DOE Badge Offices and other designated locations.
