REQUIRED DOCUMENTS
*For All Initial Claims and Redeterminations*
Required for all initial and redeterminations unless the submission is for documents requested. All DD137-3 must be fully completed. ensure that N/A or 0 is placed on anything that doesn't apply.
If parent resides in the member's household or in a dwelling owned by the member, use Fair Rental Value (FRV) for dwelling. If parent does not reside in member's household or in a dwelling owned by member, list actual mortgage, rent, or FRV if dwelling is mortgage-free. If FRV is used, give a brief explanation of how Fair Rental Value was obtained using the Remarks section. However, if parent resides in and owns home mortgage free, enter "None" in mortgage/rent/FRV block. FAIR RENTAL VALUE (FRV): FRV is a single monthly sum for the entire dwelling where the parent lives. This sum is an amount the owner can reasonably expect to receive from a stranger to rent the dwelling. FRV will not include food, utilities, furniture, and home repairs, which are listed separately. If claiming: FRV: Remarks section on DD Form 137-3 needs to state how FRV was obtained accompanied with copy of homes' ownership documents to verify who owns/responsible for home where claimed dependent resides (current dated mortgage statement, DEED, etc.). Rent: Copy of current rent/lease agreement Mortgage: Copy of current dated mortgage statement None: Copy of homes proof of ownership documents (Deed, etc.) reflecting mortgage free.
Showing parent(s) names and English translation if applicable. All changes to any part of either member or claimed dependent's name as listed on birth certificate require the legal document supporting the change.
Verification of all claimed dependents receiving income required for both initial and redetermination submissions. All income must include current year. Ineligibility for Social Security or Foreign Pension required for all claimed dependents over the age of 62. Claimed dependents who are widowed and aged 60 or older requires an ineligibility statement for survivor benefits.
Proof of support required when the claimed dependent does not live with the member or in a home owned by the member. If the home is owned by the member but the member does not live in the home ownership documents required.
