If SBP elected, enter spouse name. If no SBP election or spouse coverage, enter "NA".
If SBP elected, enter spouse SSN or date of birth. If no SBP elected or spouse coverage elected, enter "NA".
Enter dollar amount of State Tax Withholding, if no withholding enter "0".
Select state designated. If no withholding, select "NA".
If pay is direct deposited, enter bank name. If no direct deposit, enter complete check address
If pay is direct deposited, enter routing number. If no direct deposit, enter "NA".
