Report the Death of a Retiree

PLEASE NOTE:
The Name and Email fields at the top of this ticket are for YOUR information (NOT the Military Retiree's information). The Passcode field is for you to create a password for this ticket. The section below is you where need to provide YOUR contact information.
Please review this ticket in its entirety before hitting Submit. Some fields are for your information (the person reporting the death), while others are about the deceased retiree. This notification of death will only be successful if you correctly provide ALL required information.

You will receive an email when this ticket is updated. You will need to log back into askDFAS using the URL in the email. You can re-access the ticket by entering the passcode you established on this ticket. The message on the ticket will let you know if your report of death was successful. If it is unsuccessful, you will need to contact the DFAS Cleveland Customer Care Center.
Note: DFAS will stop monthly payments upon notification of death to prevent overpayment. DFAS is required to notify the bank and reclaim any payment in-full that was made after the Retiree's date of death. =====================================================================
