Information and Documentation
 
Please ensure you upload all supporting documents as they apply to you:
 
-  Your current SF 86 and/or a chronological explanation of civilian employment and gross wages earned during the re-instatement period
- IRS Form 1040s for all tax years covering the reinstatement period
- All W2s covering the reinstatement period (to include any spouse W2s for these years if filed jointly)
	- Spouse's W2s will only be used for verification purposes and will not be deducted
- Self-employment
	- The nature of employment
	- Period of employment
	- Gross wages earned
- Unemployment compensation
	- The period that compensation was received
	- Address of the office from which it was paid
- DD 214 showing original separation
- Original separation orders
- Bonus contract, if you had a Bonus contract when you separated
- Electronic Funds Transfer (EFT) Information (1199A -- Direct Deposit Form or a copy of a voided check AND driver's license)
- Department of Veterans Affairs (VA) benefits
	- The period for which the benefit was received
	- Type of benefit
	- Amount of benefits received
	- Address of the VA Regional Office that processed the benefits
- Qualifying Life-Changing Events documentation
	- If you got married during the reinstatement period:
 	     Marriage certificate
	- If you got divorced during the reinstatement period:
 	     Divorce decree
 	     Any support orders such as child support
	- If you had a baby or adopted a dependent:
 	     Birth certificate
 	     Letter from an authorized adoption agency
	- If a dependent passed away:
 	     Death certificate
	- If a dependent turned 21 and went to college:
 	     Proof of school registration
